Unbridled Rehabilitation Services is a multi-specialty therapy practice using a blend of traditional, home, and community-based approaches for occupational therapy, physical therapy, and speech therapy.

Unbridled works with individuals of all ages including veterans, disabled individuals, children, and elderly, striving to improve each person’s quality of life and functional participation to their greatest potential.

We believe that the big picture is what matters. Our team of licensed and certified professionals collaborates with your team of clinicians, caregivers, and/or educators to ensure a better quality of life.

Our Process

Everyone is unique and we are here to look at the big picture. We use a holistic approach to build a customized plan of care for each patient to achieve the best possible outcome. Getting started is a simple process and our team of professionals is here every step of the way should you need our assistance:

Step One: Contact Us

Call us or submit a form to make an appointment. We can help you determine if you need a referral before your intake appointment. 

Step Two: Paperwork

Complete and sign new patient forms and consents prior to your initial appointment. It will help you familiarize yourself with our policies and practices and gives us the information we need to handle your medical and billing records.

Step Three: Evaluation

An evaluation is an hour-long appointment with our therapist and involves interviewing, physical assessments, and an introduction to our therapies and tools. Some treatment may also be done during the initial appointment. The evaluation is a conversation between our therapist and patient or caregiver and is a great time for you to ask questions and share your history to help us build the customized plan of care to address your unique challenges and needs.

In order to sign up for any of our add-on services, the patient must be evaluated by one of URS therapists within 3 months of participation date. 

Step Four: Stepping Stones

After your initial appointment, our therapist will make a recommendation in regards to the type of therapy and frequency of care to achieve the best outcome possible for your unique needs and challenges.

Add-on services may also be discussed and suggested. Compliance and commitment are imperative to any therapeutic treatment.

You will be offered to schedule appointments at the front desk at the end of your assessment, so it is a good idea to bring your calendar and think of what times would fit into your schedule.

We offer treatment times throughout the week and on weekends. Appointments are generally scheduled for 1 month in advance. We want to be sure that you are reaching your goals and progressing, so existing patients will undergo a re-assessment by a treating/supervising therapist every 31 days at which time questions and concerns may be addressed and new goals may be set.
